Checklist - Incoming Year 1 

1. Complete the Financial Aid section in FSU College of Medicine Admissions Portal (Input SSN)
   
2. Complete 2025/2026 Free Application for Federal Student Aid* ( FAFSA) FAFSA  (School ID #001489)
   
   Please make sure you indicate that you are a graduate/professional working towards a graduate/professional degree.
   
3. Complete 2026/2027 FAFSA (Application available in October)
   
4. Accept the Student Loan offer in your myFSU portal. (Summer loan offer is in the 2025-2026 award year)
   
5. Electronically sign the Federal Direct Loan Master Promissory Note ( MPN) for Stafford Loans online at FAFSA MPN after accepting your loans.
   
6. Complete the Loan Entrance Counseling Form ( LECF) online at FAFSA LEC after accepting your loans.
   
7. Complete Student Financial Responsibility Agreementonline via To-Do Listat myFSU .
   
8. Check your To Do Listonline via myFSU for outstanding items.  If you have outstanding items, submit them to the College of Medicine Financial Aid Office.
   
9. Complete and submit the FS4U Scholarship application before June 2026. (Application available April 2026 via FS4U )

Please be advised, if items 1-8 are not completed, there WILL be at least a 2 week DELAY in you receiving your Financial Aid.
