Application Information

Applications and required documents are submitted through the ERAS system (ACGME #1271131137). If you have never registered for ERAS, you will need to visit the ERAS Fellowship Documents Office (EFDO) .

The following documents are required:

• Completed application, including most recent residency and medical school
  At least three letters of reference
  USMLE and/or COMLEX Step 1,2,3 scores

Additional Requirements:

• Must be authorized to work in the U.S. without sponsorship
• Must have completed an ACGME-accredited residency in Family Medicine, Internal Medicine, Physical Medicine and Rehabilitation, or Emergency Medicine.
